Time spent in addition to a 60 min appt: $145 for each additional 15 minutes. Times listed include time face-to-face and documentation/care coordination.

Your provider requires 48-hours notice of appointment cancellation. You are responsible for the full fee of the appointment missed, if 48-hours notice isn’t provided.

Telephone consultation, documentation, letter writing, treatment summaries, etc. are charged at the same rate as noted above ($530/hr) and prorated in 15-minute increments.

Please note that the place of service (in-office vs. telehealth) is not delineated above, since the charges are identical.

This good faith estimate explains your provider’s rate for each service provided. Your provider will collaborate with you throughout your treatment to determine how many sessions and/or services you may need to receive the greatest benefit based on your diagnosis(es)/presenting clinical concerns.
